This is a very simple concept, it's basically a set of 4mm terminals wired to the end of a USB lead. I had a bag of these cheap 4mm sockets http://www.aliexpress.com/item/50pcs-4mm-Banana-Panel-Socket-Test-Probe-Binding-Post-Nut-Plug-Jack-Connector/32563995312.html?spm=2114.13010608.0.72.OLMwwy knocking around, and I have a 4 amp USB wall charger. This thing basically provides the interface from USB power to 4mm sockets for banana plugs. Assembly is hopefully self explanatory... Just wire the red and black wires out of the USB lead to the red and black 4mm terminals. Don't forget to test with a meter before trying to connect anything. The back part was a loose fit when I printed it (I left a bit too much tolerance for the fit), it will hold in place on its own but doesn't have a firm snap - so adjust if this bothers you. As a 'stretch goal' you could install an LED behind the 5V text on the front, the layers are thin enough to let some light through. Just wire across red/black along with a suitable resistor and you should have a power on indicator.
