Basement Floor Drain Plate w/ HVAC condensate drain holes

My high efficiency gas furnace and AC unit has drains for combustion condensate, humidifier overflow, and AC a-coil condensate that when originally installed by the HVAC contractor were simply zip-tied to the cast iron floor drain grate. The condensate would run back along the tubing laying on the floor and make a mess. The concrete was always wet, and slim would form. It even started to deteriorate the concrete. This is just a replacement grate with appropriate holes to allow the drain lines to be fitted with elbows to direct the water into the drain with no water running back along the tubing. I also included a one hole version that I created for a friend who had a similar situation, but only had one, 1" PVC pipe as the drain from the HVAC unit. The diameter of grate is a standard 6.5" basement floor drain.
