This is an auto-bed level attachment for the Solidoodle Bulldog XL E3D carriage by wardjr. It uses the Escher3D opto end stop, https://miscsolutions.wordpress.com/mini-height-sensor-board/ wardjr was nice enough awhile back to add a couple holes in the front of the carriage to mount the bed leveler too.
