There is plenty of Clean/Dirty signs available on the internet, but neither fulfilled my needs - survive one-year-old running around. The key features of this design:

Printed in-place as a single item - e.g. no small parts that curios child can take apart.

No glue on the surface of the model that could be accidentally licked.

All magnets are safely stored inside the model and can't be taken out.

The "mover" is held on both sides of the model by magnets too. Meaning you have to use a bit of intentional force to switch from "Clean" to "Dirty" and vice-versa. Purely twisting the model around or upside down won't switch the state (the one-year-old is yet to figure out how to switch the sign).

You will need 18 cylindrical magnets 6x3mm. I had few leftovers from my past project bought here:

https://www.aliexpress.com/item/32770059462.html

The shape actually isn't entirely ideal and requires a careful installation of the magnets on the second layer (see printing instructions).

Print instructions

The easiest is to use the provided gcode model that already has all the required configuration - it will pause the print twice for you to insert the magnets. It's built for MMU2S using the first three filament slots:

Filament #1: The box itself (suggested color black)

Filament #2: "Dirty" text (suggested color red)

Filament #3: "Clean" (suggested color green)

Feel free to slice the model yourself though, all the model parts are attached.

Inserting magnets:

There are two layers of the magnets - the first layer to attach the sign where you need to and second layer that helps hold the "mover" in place (preventing accidental state switch).

Layer #1: This layer is actually straightforward - just insert the magnets when the printer asks you to. They will be safely held in place by the strong magnets in the print plate. No glue necessary.

Layer #2: Sadly this layer is too far from the print plate and if you won't glue the magnets, they will jump out during printing (the nozzle itself is made from metal). Since the magnets are cylindrical, it's crucial to give them the right orientation while gluing them. This isn't as hard as it seems - just create a line of magnets, mark their orientation with a marker like on the pictures and then insert one magnet to each space. Attach 2-3 magnets to the side of each space to force the magnet inside to properly align (see the pictures). Then just put some glue on top and wait 5-10 minutes. Remove the extra magnets and let the print continue. Don't go overboard with the glue - otherwise, the nozzle will push excessive glue to the side of the model.
