I was inspired to build this by Prusajr and his PCB heated bed, http://www.thingiverse.com/thing:2172 . It is designed to be etched at home using the laser printer toner method on a standard Radio Shack 11.4mm x 16.1mm PCB. It is about 2.9 ohms at 12 volts and heats up to 110C in about 4 minutes. It has a spot for a SMD thermistor, but i am not sure if the pad spacing is correct as I do not currently have one. If it is not correct, it shouldn't be too hard to fix. Instructions For instructions on etching your PCB using the toner method, go to http://www.instructables.com/id/Cheap-and-Easy-Toner-Transfer-for-PCB-Making/ . The PDF is already correctly formatted and mirrored for printing and is the easiest way to print it out. If you want to make any changes you can open the BRD file in eagle.
