remove the silver frame (clips from bottom, with small flat screwdriver or knife). Then preheat the extender slightly (! so you can still hold it in your hand) and carefully slide over the throttle without braking it off. If its tight remove, reheat, repeat. Slide something in between the handlebar and the throttle to brace and prevent braking off. If succeeded sliding on, reheat slightly again the bridge of the extender and push it down in place. This way the part will not slip down. I hope it makes sense. Added lower thumb position version.
