This is a remix of https://www.thingiverse.com/thing:3279208 I don't trust myself to glue 3 pieces together, so I gave it a try to print the whole adapter together. The stls combine the Bottom + Spacer 4.5 with slot + Top v3. The bottom fits just right. I'm still waiting for my lens to arrive, so will see if the lens fit the top. But I assume it will since the original design is very error tolerant. Updated: I inserted the lenses and it works perfectly for me.
