This is a tailpiece adapter for the Level electronic lock. The adapter fits the tailpiece for a Schlage B62 dual cylinder lock. Just be aware that the door likely needs to be very thick in order for the lock to work - two cylinders plus the Level unit take up too much space for a standard door.
