The model has been created in 3DS Max 2012, it is available in different format (obj, fbx, max and 3ds ) along with its textures and uv layouts. It is not exactly high poly nor low poly but rather in the middle of both. The assets have been renamed and organized accordingly in layers providing a clean workflow. Vray has been used in all materials and for rendering. In total it is 2001 polygons. It can be used in your game or any creation of yours.
