This is a modified version of the Makerbot Replicator extruder that I am planning to install on my Printbox. Uses two standard Printbox3D hotends and coolers. The extruder is redesigned to use MK8 driving gear and a 604Z bearing held in place by a 4x10 mm screw. Also, the design has been minimized and now the whole thing (including the lever) is just the size of a NEMA17 motor. And yeah, it can handle flexible filaments. You can mirror the parts and easily make a double extruder.
