I printed up a duplicate of this fan adapter by Area51 twice, convinced I had botched my settings somehow. It turned out the fan screws were threaded at M2.5 instead of M3. So I tweaked the step file, carved out the fan screws at M3, and added threading in Fusion 360 to make my fan screws lock into place. When wiring up the new fan to the old adapter, using the Noctua extension cable that comes with the fan proved a wise decision. This allowed me to unplug and adjust the fan wire as needed, making it ridiculously easy to track down any issues.
