This should be a replacement for a round phone jack plate. I have these round phone jacks in my 1970's house. They don't seem to be made exactly like this anymore. I just want to cover these receptacles up which is why I originally created the blank. I then created one with a hole for completeness. Holes are 47 mm or approximately 1.85 inches (1 7/8'') apart.
