This is a simpler version of the original PALS Armor holder made by Juggernaut. It would fit your phone just fine. In addition to 3d print you will also need some kind of pin, or a 4mm diameter rod to create a hinge. Also, to assemble the smaller hinge correctly, you will need a threaded insert and an M5 12mm bolt.
