This is a support for linear bearing KH1428PP, made by measuring the dimensions of a real one, which is hard to find. Instructions Print the part You will probably need to adjust a little the 21mm hole to be able to push the bearing into it by hand (tight fit, use paper sand) add two 6mm nuts (see pictures) . You will also need two M6 screw. Et voilà !
