A very simple way to fix a mounting plate onto the index, use a small piece of double sided tape to place the mount then secure with a R-31 O-Ring. Mount has a un-tapped hole that accepts 1/4" screws (such as a camera mount), threads are created during insert. Just thought I'd add, if you wipe the 3d printed part with acetone (NOT THE CONTROLLER) before adding the double sided tape it sticks far better. Also instead of using the o-ring I've found its better to use amalgamating tape to secure it. Its very solid and a good strong base. Also I forgot to mention that I made this part with the hope that it will break before your controller, I've broken a few of these connectors hitting things and so far the controllers are just fine.
