Wheel adapter for 100mm Nexus Mechanum wheels (https://www.robotshop.com/en/100mm-mecanum-wheel-set.html) to Rev. We use the Rev Hub Strengtheners on the inside of each plate http://www.revrobotics.com/rev-41-1147/ There's plenty of room for the hub strengthener to be on the inside of the plate so the entire assembly isn't much wider than the wheels themselves. These can be laser cut, 3d printed, or machined.
